If you are a new patient, please arrive 20 minutes prior to your scheduled appointment. In order to expedite your visit, please login to your patient portal to fill out your new patient forms prior to your arrival. If you are unable to complete the forms online, you may download the forms below and fill them out prior to your visit. Spending the appropriate time with our patients is of the utmost importance to us as we strive to be a top-tier Goodyear OB/GYN provider. In pursuit of this, if you are more than 15 minutes late for your scheduled appointment, you may be asked to reschedule your visit.
